This public website provides professional and educational information. The website itself does not accept referral details or documents and does not collect workbook answers. It links to separate Microsoft-hosted referral services. The website does not use analytics.

Routine website records

The hosting service may create ordinary access and security logs when a page or protected workbook file is requested. These records can include the date and time, requested address, internet protocol address, browser information and security events. They do not contain anything typed into a workbook activity because workbook answers are not sent to the server.

Clinical referral collection notice

The website links to two separate services in Recovery Well Medical Services' Microsoft 365 environment. Microsoft Forms collects current patient and submitter contact and matching details so the practice can receive, triage and respond to a referral. A separate SharePoint File Request receives referral documents. The two submissions are not automatically linked and are matched manually by authorised practice staff.

The practice uses the submitted information to assess and respond to the referral, contact the relevant people and maintain appropriate clinical records. Access is limited to authorised practice personnel, and the information is managed under the practice's record-handling requirements.

Provide referral information only if you are authorised to do so. The form is not the place for a diagnosis or detailed clinical history; put relevant clinical information in the referral document. For access, correction or a privacy question about submitted information, call the practice on (03) 9526 0244.

Referring doctors may instead send referral information and relevant documents to the published clinical referral email or fax. These Microsoft, email and fax channels are separate from the public website, and the website does not receive or store the referral.

The referral email is an automated workflow for receiving and processing clinical referrals and relevant documents. Do not send clinical requests, administrative requests or urgent messages to that inbox. The form, upload page, email and fax are not continuously monitored. Call the practice for clinical or administrative requests. For a medical emergency, call 000 or attend the nearest emergency department.

Independent expert correspondence

The dedicated independent expert mailbox is separate from this website and is strictly for a brief, non-confidential suitability enquiry from an instructing organisation about a proposed formal independent medical expert engagement. Dr Hadinata accepts an instruction only where he has no current or previous treating relationship with the person to be assessed and no other conflict of interest. The mailbox is not for accepted or current matters, general legal or insurance enquiries, patients, members of the public, clinical referrals or ordinary report requests. Do not send instructions, party names, briefs, health records or other confidential matter information by email. MEDirect handles all accepted or current matters through its specialised medicolegal IT services and risk-management processes. Messages outside the preliminary-enquiry scope are automatically removed before human review.

Workbook answers

Workbook answers remain only in the active page. They are not submitted or saved by the website. Refreshing or leaving a chapter clears active answers. Printing or saving creates a copy controlled by the person using the device.

Information this website does not accept

Do not enter patient details, referral files, health records or confidential matter documents anywhere on the public site. If you create a printed or saved workbook copy, you control that copy and should store it safely.
