Check the referral before uploading
Please make sure the referral itself clearly includes the patient's current full name, date of birth, address, phone number and email address, together with the referring practitioner's current name, practice, phone number and email address.

For normal processing, upload PDF, DOCX, JPG or JPEG, PNG, HEIC or HEIF, and TIF or TIFF, up to 15 MB per file. If a file is different or larger, use email or fax instead.
