Clinical referral pathway
Send a clinical referral
Use the two-step online referral pathway, or send the referral by email or fax.
Use this pathway only to submit a clinical referral and relevant documents. For general, clinical or administrative enquiries, call (03) 9526 0244.
Online referral
Complete both steps. Each opens on Microsoft's website in a new tab so you can return here for the next step.
Referral submissions only
Do not use the details form or document upload for general, clinical or administrative enquiries. Call reception on (03) 9526 0244.
Before continuing, read the clinical referral collection notice.
Provide current contact and matching details
Enter the patient's current details. Do not enter a diagnosis or clinical history in this form.
Open the details formUpload the referral documents
For normal processing, upload PDF, DOCX, JPG or JPEG, PNG, HEIC or HEIF, and TIF or TIFF, up to 15 MB per file. If a file is different or larger, use email or fax instead.
Open the document upload
The two steps are matched manually
The details form and document upload are separate submissions and are not automatically matched by name or time. Completing them does not confirm acceptance or an appointment.
This public website does not receive or store the information. The buttons open the two separate referral services.
Email and fax alternatives
Fax
Fax the written referral and relevant documents to this number.
Include the reason for referral and the questions you would like addressed. Most supporting information is optional and can follow if needed.
The practice aims to triage referrals and respond to appointment requests within one week. During busy periods, this may take a little longer. Sending a referral does not confirm acceptance or an appointment.
